Dental Referral Criteria
Who can refer
Physicians, Dentists and Nurse Practitioners
Patient age
- Patient must be under 18 years of age.
- Patients who are enrolled in the Ontario Cleft Lip and Palate Craniofacial Dental Program may be referred up to 22 years of age.
Patient acceptance
- Children with a significantly compromising medical condition such as cancer, organ transplant, or complex heart disease
- Children registered on the Ontario Cleft Lip and Palate Craniofacial Dental Program
- Children with severe dental caries and significant medically diagnosed developmental disabilities
- Children with severe early childhood caries up to age 3
- Children between 3 and 5 years of age with severe early childhood caries whose treatment needs align with the requirements of our educational program in advanced pediatric dentistry
- Children with acute dental emergencies
Rejection criteria
- Referrals for examinations, x-rays and/or cleanings under general anaesthesia or sedation
- Referrals for management of incipient or clinically minimal caries that can be managed alternatively

Requests for a second opinion unless submitted by a paediatric dentist with a documented clinical rationale

Treatment cost
Our clinic follows the Ontario Dental Association’s Suggested Fee Guide for Paediatric Dentists.
Patients and families or caregivers are responsible for all costs associated with dental treatment in our clinic.
If you have extended health or dental insurance, please provide your insurance information to the dental clerks at your first appointment. A dental plan can help cover some of the costs associated with dental treatment.
Families are encouraged to apply for the CDCP (Canadian Dental Care Plan) early as this program can reduce with dental care costs on its own or in combination with other coverage. Information can be found here: Canadian Dental Care Plan eligibility and application.
Coming to your appointment:
Please arrive 20 minutes prior to your appointment.
Please note that patients who arrive late face appointment rescheduling or being seen at the end of the day to avoid disrupting the clinic schedule.
Required supporting documentation
Specify treatment needed (restorations, extractions, pulp therapy). Incomplete referrals will not be triaged.
